Personal data may have been exposed on Chicago health agency dashboard
CHICAGO - Certain personal information may have been inadvertently exposed on a Chicago government dashboard detailing public health information last year.
The Chicago Department of Public Health posted a notice of the event on its website on Tuesday.

What we know:
Around Oct. 8 of last year, the public health agency learned about the potential disclosure of sensitive information.
The information was used to generate statistics related to public health and safety, according to the CDPH.
The agency began an investigation into what happened and determined that certain personal information, including certain individual’s names and medical information, could have been accessed on the public dashboard.
Officials were not aware of any actual or attempted misuse of personal information. CDPH said it took action to reduce the risk of similar events happening in the future.
All potential public access to the personal information has been disabled, according to CDPH.

The CDPH said as an added precaution, it will offer 12 months of complimentary access to credit monitoring services through IDX, a ZeroFox company, to individuals who were affected by the disclosure.
The agency urged the public to remain vigilant for possible identify theft and fraud by reviewing their account statements and monitor their free credit reports for suspicious activity or errors over the next 12 to 24 months.
